Q1. Which patient with the following manifestations is most likely to develop hypercapnic respiratory failure?
Large airway resistance as a result of severe bronchospasm
Slow, shallow respirations as a result of sedative overdose
Rapid, deep respirations in response to pneumonia
Poorly ventilated areas of the lung caused by pulmonary edema
Show answer and explanation
Answer: B. Slow, shallow respirations as a result of sedative overdose
Hypercapnic respiratory failure is associated with alveolar hypoventilation with increases in alveolar and arterial carbon dioxide (CO2) and often is caused by problems outside the lungs. A patient with slow, shallow respirations is not exchanging enough gas volume to eliminate CO2. Deep, rapid respirations reflect hyperventilation and often accompany lung problems that cause hypoxemic respiratory failure. Large airway resistance and pulmonary edema cause obstruction of oxygenation and result in a V/Q mismatch or shunt typical of hypoxemic respiratory failure.
Q2. The patient diagnosed with a community-acquired pneumonia is being admitted to the unit. Which doctor's order should be implemented first?
Place patient in respiratory isolation
Obtain a sputum specimen for culture and sensitivity
Administer Rocephin 50 mg IVPB every 24 hours
Apply oxygen 2 L via nasal cannula
Show answer and explanation
Answer: B. Obtain a sputum specimen for culture and sensitivity
A. The nurse should administer the prescribed antibiotic as soon as possible but not before obtaining sputum culture. B. The patient with pneumonia needs more than 2 L oxygen via nasal cannula, which would be appropriate for a patient diagnosed with chronic obstructive pulmonary disease. C. The nurse needs to obtain a sputum culture before administering antibiotic because the culture and sensitivity will be skewed if the patient receives antibiotics. This should be the first HCP implemented. D. The patient with pneumonia is not placed in respiratory isolation. This HCP order should be questioned.
