Sample Heart Failure questions

Q1. Nurse Bea is assessing a male patient with heart failure. The breath sounds commonly auscultated in patients with heart failure are:

  1. Friction rubs
  2. Fine crackles
  3. Tracheal
  4. Coarse crackles
Show answer and explanation

Answer: B. Fine crackles

Fine crackles are caused by fluid in the alveoli and commonly occur in patients with heart failure. Tracheal breath sounds are auscultated over the trachea. Coarse crackles are caused by secretion accumulation in the airways. Friction rubs occur with pleural inflammation.

Q2. The nurse is preparing a patient to be discharged after a new diagnosis of heart failure. Which statements by the patient shows an appropriate understanding of the nurse's teaching?

  1. "I will call my doctor if I gain more than 2 pounds in a day."
  2. "I will take my angiotensin-converting enzyme (ACE) inhibitor as needed for shortness of breath."
  3. "I will do weekly finger-stick monitoring of my sodium levels."
  4. "I will not take my diuretic pill on weekends when I am traveling, to avoid incontinence."
Show answer and explanation

Answer: A. "I will call my doctor if I gain more than 2 pounds in a day."

While finger-stick glucose levels are done by patients to monitor their diabetes, sodium fingerstick levels are not done. ACE inhibitors need to be taken daily, as prescribed (not p.r.n.). Diuretic pills may be delayed a few hours before a big event, but not skipped or the patient may end up with a heart failure exacerbation. A patient who calls about a sudden weight gain may receive instructions from the doctor to come in to be evaluated or may be instructed to take an extra diuretic pill at home.
